How to Register with the Surgery

We have an open registration policy where anyone living within our practice boundary that requests registration with the practice will be registered.

Please come in and fill in the required registration forms. Once completed, please make an appointment for a new patient medical assessment. If you are on medication this assessment should be with a doctor; if you are not on medication, with a health care assistant. Please bring a urine sample with you for this appointment.

Please download the GMS1 form and complete and return to the surgery to register.
